SPECTRUM QUEER MEDIA • VOLUNTEER ARTIVIST RELEASE FORM
Please read and fill in. A hard copy will also be provided for you to sign the day of your performance/exhibit.
The Artivist, listed below, agrees to hold Spectrum Queer Media and its Production Committee herein referred to as The Organizers) harmless from all claims arising from the use and promotion thereof in kind, of all kind of any kind whatsoever. The Organizers do not guarantee or warrant any such claims to the success or failure of Artivist participation.
The Artivist acknowledges their participation as a volunteer for a SPECTRUM QUEER MEDIA event (herein referred to as the Event), upon the terms and conditions contained in this Agreement. The Artivist's execution and return of this Agreement to the Organizers is a condition precedent to Artivist’s volunteering for the Event.
The Artivist acknowledges and agrees that he/she is not an employee, agent, or contractor of the Event. Volunteer further acknowledges that he/she has received no promise regarding and has no expectation of compensation of any sort and that he/she will not become eligible for any employment benefits as a result of the volunteer activities that he/she performs under this Agreement. The Artivist is agreeing to perform volunteer activities solely for his/her personal purposes which includes a desire to support the betterment of the community.
The Artivist also grants the Organizer the right to use the Artivist's name, voice, image of physical likeness, biographical material, contact information, and any other information, on the Spectrum Queer Media website, relevant social media pages and any other multimedia application used in manner of any kind whatsoever.
The Artivist agrees to check in at the Event entrance approximately 30 minutes prior to the scheduled event doors open start time.
As a volunteer, subject to the terms of this Agreement, the Artivist will receive the following privileges:

one (1) commemorative flyer upon check in on the day of performance.

one (1) Artvist pass + 2 guest passes granting admission as an Event Artvist and Artivist Supporter.

space on a shared Artivist merchandise table to sell/share promotional material/merchandise
